Cam wore it correctly... at least in the "rules" of fashion. You're not supposed to button all of the buttons of a double-breasted jacket.

Does this apply to Pee coats too? What if it's bloody cold out?

First of all, it's outerwear, so it's not subject to the "rule" you're talking about.
And again, there's no unanimity when it comes to buttoning double-breasted jackets.

Do you honestly think leaving one button undone will make a difference?
If it's cold and really windy, maybe, but otherwise I don't think it would, especially considering that you would probably be wearing other layers underneath.
I think buttoning all the buttons would look too stuffy
